Education Social Service Facilitators

The mission of Education Social Service Facilitators is to assist and work with children and their families to improve each child’s academic success by providing intervention, assistance, and on-going support utilizing a wraparound, strength-based model in coordination with the Community’s resources.

Education Social Service Facilitators provide the following supports and services:

  • Advocacy
  • Case management
  • Academic support
  • Whole family support
  • Mentorship
  • Referral, support, and connection with tribal and stateside resources, including
    • SRPMIC Child Protection Services and Social Services
    • Phoenix Indian Medical Center (IHS), Salt River Clinic, Diabetes Program, Phoenix Children’s Hospital
    • Touchstone Behavioral Health, Southwest Health Services, Arizona Children’s Association
