Sign your student up for their flu vaccination, which will be conveniently provided at the different school sites beginning the week of Oct. 22.

Make sure to return your child's consent form ASAP so they can get their flu shot right at the school!

The flu shot clinic will be provided on the following days/locations:

• SRES on 10/22 (480-362-2400)
• SRHS on 10/23 (480-362-2000)
• ECEC on 10/24 (480-362-2200)
• ALA on 10/25 (480-362-2130)

Flu vaccination consent forms are also available at your child's school and the Salt River Schools administration building.
