The mission of Education Social Service Facilitators is to assist and work with children and their families to improve each child’s academic success by providing intervention, assistance, and on-going support utilizing a wraparound, strength-based model in coordination with the Community’s resources.
Education Social Service Facilitators provide the following supports and services:
- Advocacy
- Case management
- Academic support
- Whole family support
- Mentorship
- Referral, support, and connection with tribal and stateside resources, including
- SRPMIC Child Protection Services and Social Services
- Phoenix Indian Medical Center (IHS), Salt River Clinic, Diabetes Program, Phoenix Children’s Hospital
- Touchstone Behavioral Health, Southwest Health Services, Arizona Children’s Association
